Session notes

Support session notes, written properly, every visit

Every support session should leave a clear record: what the resident needed, what the support worker did, how the resident responded and what was agreed next. That is what councils and landlords read when they check the support behind a claim. It is also the part that gets written late at night, in a hurry, or not at all.

How it works

  1. 01

Your support worker follows a short guided form on their phone during the visit, about five minutes, mostly taps.

  2. 02

Our team writes the full session note on your own template, using the resident's history and your house style.

  3. 03

A second person checks every note before it is released.

  4. 04

The finished note arrives in your portal as PDF and editable Word, within our 24 working hour turnaround.

What a good session note shows

  • ·The date, the time, who delivered the session and how
  • ·The concern or goal discussed, and how it links to the support plan
  • ·What support was given, in plain words
  • ·How the resident responded, in their own view
  • ·What was achieved or agreed for next time

Nothing invented

If something is unclear, we ask the support worker who was there. Gaps are queried, never guessed. The original submission is kept exactly as sent.
